Job SummaryUnder general supervision, and according to established policies and procedures, responsible for the provision of
specialized transition of care clinical pharmacy services to patients, nurses and medical staff. Collaborates with other health care professionals in the research, development and implementation of critical pathways and policies that have a medication
use component. Practice involves the components of expert practitioner, educator, consultant, researcher, and clinical
leader. Responsible for performing job duties in accordance with the mission, vision and values of Tampa General
Hospital.
Specific Duties Include, But Not Limited To
Serve as pharmacy resource for the multidisciplinary transitions of care team
Ensure medication safety across care transitions including admission, discharge, and post-discharge
Provide patient education and medication reconciliation post-discharge
Utilize evidence based guidelines to optimize medication therapy and improve patient outcomes throughout care transitions
Pharmacy degree (Pharm. D or Ph.D.) from an accredited college of pharmacy and completion of an advanced
residency, fellowship, or equivalent experience.
PGY2 in Ambulatory Care and/or transitions of care experience preferred
Current license as a Registered Pharmacist in the state of Florida.
